Any person, sixty-five or older, who has been a resident of the Greater Lynn Area for at least five (5) years during their lifetime is eligible to apply for admission to Lynn Home for Elderly Persons ("LHEP").
LHEP may only accept or retain an individual who applies for admission to LHEP ("Applicant”) if the Administrator determines that LHEP is able to provide appropriate services to Applicant and Applicant meets the requirements set forth by state law. LHEP is not required to admit or retain Applicant, or to contract with Applicant for services, if LHEP determines, in its sole discretion, that LHEP cannot meet Applicant’s needs or Applicant fails to meet the requirements set forth by state law.
Residents are admitted to LHEP without regard to race, color, creed, national origin, sex, sexual orientation, or religion.
Applicant must contact the Administrator who will interview and conduct an assessment of Applicant. The Administrator will explain admission requirements and procedures. If the Administrator determines that Applicant may be appropriate for admission, then the Administrator may give Applicant a tour of LHEP.
The Administrator’s Assessment includes an evaluation of each of Applicant’s specific needs. It covers areas such as mobility, skin care, eating habits, oral hygiene, continence, cognitive behavior and medication.
If the Administrator is satisfied that Applicant should proceed with the admissions process and Applicant wishes to proceed, then the Administrator will give Applicant a copy of the following:
Applicant must complete these forms and return them to the Administrator with a non-refundable Two Hundred and Fifty Dollar ($250.00) processing fee.
The Administrator will forward the Application for Admission and the Financial Report Form to the Admissions Committee of the Board (the "Admissions Committee") and forward the Medical History Form to the Medical Director and the Nurse Manager. After review of these forms, the Administrator in consultation with the Admissions Committee, Medical Director and Nurse Manager will determine:
Applicants are required to have a physical examination by LHEP’s Medical Director and an assessment by the Nurse Manager. In order to proceed with admission, the Medical Director’s examination must demonstrate that Applicant meets the medical criteria for residency.
The Medical Director’s evaluation and the Nurse Manager's Assessment provide a basis for determining Applicant’s need for services.
Applicants are required to meet with the Board’s Admissions Committee. The Admissions Committee may visit Applicant in Applicant's home or meet with Applicant at LHEP.
If the Administrator, based on all the information gathered during the admissions process and in consultation with the Medical Director, Nurse Manager, and Admissions Committee, determines that Applicant does not meet the criteria for admission, the Administrator will notify Applicant of that determination.
If, the Administrator, based on all the information gathered during the admissions process and in consultation with the Medical Director, Nurse Manager and Admissions Committee, determines that Applicant does meet the criteria for admission, the Administrator will notify Applicant of that determination. The Administrator will also advise Applicant of the financial and residency requirements for their admission. Applicant must agree to these requirements before admission to LHEP.
If, in accordance with paragraph H above, the Administrator has determined that Applicant meets the criteria for admission, but there is no current vacancy at LHEP, Applicant will be placed on a waiting list. Subsequent vacancies at LHEP will be filled from the waiting list.
If the Administrator, in accordance with paragraph H above, has determined that Applicant meets the criteria for admission and there is a current vacancy, the Administrator will provide Applicant with a Residency Agreement and review the Agreement with Applicant. Prior to Admission, Applicant must sign the Residency Agreement. Once Applicant has signed the Agreement, the Administrator will set an Occupancy Date and, barring unforeseen circumstances, Applicant will become a Resident of LHEP on that date. The terms of the Residency Agreement take effect on the date Applicant and LHEP execute the Residency Agreement and will remain is effect throughout the admitted Applicant's residency at LHEP.
